would like to smack this author for incorrectly placing his commas OUTSIDE the closing quotation marks (the dialogue tags come at the end).

The author is inadvertently using the British style instead of the American style - mainly because he's bad at punctuation. The author is from the U.S. and is writing a story that appears to mainly be set in the U.S. (kind of). I'm pretty sure that he's not even aware that there's a British style.

I am again starting to feel as if a number of my authors are just clueless chimps who punch keys at random and don't pay attention to what they're doing. :P
