Medium-high is what I've got. Did you disable the origin overlay? It helps with performance drops.

The hinterlands section is really long and boring, as soon as you've got enough power I'd recommend moving onto the story missions, they're way more enjoyable.

can't remember the exact names of those option (right now Civilization is running!), but my Overall texture and face and character graphic options are at ultra, the rest are medium-high. Note: Low shadow

I think it's quite great for a Corei5 and graphic card made in 2009.
Yep, killing the first huge demon was hard for me at the first try.
