So I've had this problem only with Wynne, Morrigan and Loghain in the last couple of games I've played.
They all run about town with their weapon drawn, as though in combat. This only happens intermittently so I don't have a clue what's going on.
In my last cleared game, Loghain would run around as though in combat with sword and shield drawn regardless of where we were, but there'd be no problem with him in battle. Same for Morrigan.
However, in my last new game, Wynne has started running about as though in combat as well. Except this time she always insisted on waiting until we were actually IN combat to run off and die somewhere LOL
When we were in the Brescillian Forest, fighting the Denerim mercs, she ran off. Half of them followed her and she died right after running so far off that she triggered the Swiftrunner cutscene!
When I went to get Shale, she did exactly the same thing except the map was totally cleared (this is after activating Shale). She just ran off to the other side of the map for no reason :|
Whuuuuuuuuuuut? Nobody else has run off to die, but them running around in such a manner is really irritating - especially as it means they are far behind the rest of us. And it's just weird that it's only these characters, and only Wynne goes off to die. XD

It is usually caused by a cutscene that activates at the end of combat...but before everyone's weapons are put away. Then the combat mode never stops.
To stop it, I have found that all I have to do is save the game, and then reload that save. Don't have to exit the game or anything. Loading the save resets people to the state they should have been at.

I think that's what happened with Loghain, but he didn't stop semi-squatting until 2 reloads later, and reloads never save Wynne o_O! Everyone else is ok though!

Have you tried switching weapon sets? I can't remember if that fixed my particular issue with that. Also, activating a spell almost always works.
