I posted my problem in the french forum but i come here to explain my problem (hoping that a developer sees my message).

So I just saw a bug and I just understood it.

I run my game on the suicide mission with a commando miranda and Thane against Occulus (for their warp) except that once the fight starts I start using warp with my whole team on the enemy, except that the error message "disk unreadable" appear and the game restart.
This message is totally weird because my game is installed on my 360's hard drive (so no pb in relation with the drive normally) so I retries twice to see if it was casual but nothing change, as soon as I launch the attack the error message appear.
So I do my tests and discovers that when there is only me or miranda who are launching our warp no problem but when I run it with Thane the error occur, so I began to think and what which has changed since the last time I finished the game, his outfit, so I uninstall the pack and held it as if by magic it works there, of course I tried again several times with and without the pack and now I'm sure that my deduction is correct.

So outfits for which we pay are crashing the game, then I am told that it happens in the suicide mission, but nothing says that it can happen to many other time before.
So I leave you my experiment to see if I am alone but if I'm not it's a really big error, so I wish developers (at least one) will see my message and fix this problem, but it would be really cool it there are some thing done quickly because I paid for this content.

I'll keep trying with the other squad members (Garrus and Jack) but I think it will be the same.

Sorry for my english i'm french ;-)
