It was intentional. I wanted to show ALL of the potential squad mates.

Except for Liara, any of them can be dead. (Okay, Joker and Chakwas are safe too, but they're technically not squadmates either)

Since I originally made that composite to be a desktop image, it makes it interchangable for anyone to enjoy no matter who they left on Virmire or whether they killed Wrex, sold Legion, ect.  It's also why I put Shepard in a helmet rather than having my Shepard's custom face in the picture.

It was the Gyroscope thing that made me mention 13 ghosts. Coould be wrong though, haven't seen Event Horizon.
