What moment in the Mass Effect series made you feel like a giant idiot?

When I let the young frightened Asari commando go on Illium after listening to her story about how she got caught up with the wrong crowd... then finding that audio recording later in the level with her bragging about slaughtering innocents and enjoying the massacre.
Yeah, I blow her away every time now, even on paragon

Yeah, that plus the triple headshot is always awesome. First time I saw that I was like

Letting Elnora go, then finding out she was the one who killed the volus on Illium. Despite knowing this, I still let her live on my subsequent playthroughs.
This. I have never felt like a bigger idiot, especially since everyone had been telling you that each Eclipse sister commits a murder to earn their uniform. How ever, now I just always kill her.
I shall not be taken for a fool. Not again.
Also, I don't know why, but I got the whole 'Harbinger is actually a Reaper' thing first time around.

Also when Morinth was like "Hey I am going to reward this conversation with a couple of drinks, be right back"
"Oh ok." *5 Minutes Later* Samara comes by and tells me I messed up..
Another one in Mass Effect 1 was trying to figure out how to get back to the Normandy after landing on an exploration planet. Spent a good 2-3 hours on that...
ME2 solo mission where you have to explore a shipwreck. Needed to watch a walkthrough to discover you needed to shoot down debri to use as a bridge.
..Thinking the upgrades to the ship wouldn't make a big difference to the outcome of my suicide mission... epic fail.
Never realised until the end of my first planet on Noveria that I could upgrade my skills with points. I would say to myself that this was the hardest game ever, and eventually discovered it after not being able to beat Benezia without it.
Me going into the Mass Effect 2 meeting with the alien council thinking, nice they are going to love me and totally believe me about the reapers. "Ah yes, WRONG!"
My brother: "Time doesn't really exist in Mass Effect 2, so I can go have a few drinks at the Dark Star before I go save the crew."

The space hamster clued me into the bathroom ... on my second playthrough. I held it for a reeeeaaalllly long time the first time around.
Spending money on all those pretty fish only to return triumphant from the suicide mission to a tank full of dead fish. Guess I didn't save *everyone*.
Discovering that medi-gel not only revives dead squadmates but can also heal you if you're low health. *facepalm*
Letting Elnora go the first time. I could just see Miranda rolling her eyes at me.

Oh for god's sake. I'm a retard. I'm currently on my fifth playthrough and I always thought that the Trauma Module upgrade lets you revive your team members to full health. Like when it's not installed, they would only gain 75% of their health upon reviving. But indeed, it actually lets you heal yourself (and your squad mates). Dammit.

Realizing, on my third playthrough, that the "vent technician" during the first leg of the suicide mission, can be seen progressing through the transparent tube above you and to your left, the whole way through.
REALLY??
I've completed the game 4 times and I've never noticed it...
