Getting to my crew on the Collector ship on my first playthrough , in time to see them melt. The reason for me taking my time : I was buying fish , scanning planets and talking to random people , sight-seeing basically. Ouch.

Also getting lost on Illos everytime I do an ME1 playthrough I can never figure out where to go.
The first time I played ME1 after I unlocked the security door I ran around Illos looking for the Mako when it was right infront of me.

Oh for god's sake. I'm a retard. I'm currently on my fifth playthrough and I always thought that the Trauma Module upgrade lets you revive your team members to full health. Like when it's not installed, they would only gain 75% of their health upon reviving. But indeed, it actually lets you heal yourself (and your squad mates). Dammit.
RIGHT NOW I'm feeling like a giant idiot. Thousands playthroughs, only to find out now.

In ME2 finding the bathroom by mistake—near the end of my second playthrough. And not realizing that there where shutter controls in the cockpit until I watched a video on youtube where the guy started fooling around with them near the end of the video.

I can top that, even. Somebody actually posted a screenshot of that line in the motivational thread and I, not realizing such a joke was already present, said "Well at least he's not considering transferring to the deferens", thinking myself reasonably clever.....until I got slammed with the force of my own stupid.

Realizing, on my third playthrough, that the "vent technician" during the first leg of the suicide mission, can be seen progressing through the transparent tube above you and to your left, the whole way through.
Initally, I thought it was Collector troops being deployed to join the fight:
"Oooh, that's a nice touch. You can see Collectors milling about in the tubes"
and, a moment later...
"Whoops! Scrath that. It's Legion."
Sometime, I just forget to look up.
Also, on my first playthrough of ME1, it took me forever to find my way back to the C-Sec elevator that led up to the Normandy so I could get the Hell off the station. Off to a brilliant career as an elite defender of galactic stability, eh...
