For me it was 'Bring down the sky' I'm a paragon player but this was one of the few instances where letting the Bad guy go was unacceptable. I still felt like crap when I saw the burnt corpses of the hostages but he had to be captured. I still didn't kill him but I did shoot him and leave him for the alliance to take care of. I did try the other way but I still felt unsatisfied, that's why this was the hardest decision in both games for me because neither option felt good.  I picked one over the other simply because in this instance I was dealing with a diabolical terrorist  and not some loser merc  that you wouldn't see again if you scared him enough.

This one sucked, and so did the Batarian crisis in the ME2 side mission. You either save the small colony and ruin expansion plans or sacrifice them. That was harder to decide than the choice to destroy the collector base or hand it over to TIM for me. I also hate Virmire, I like Ash and Kaidan both.

Oh, that Baterian one killed me. I took about 10 minutes to decide! (seriosuly, I made a sandwich and thought about it).

The 'Kill Sidonus' one always got to me. I guess because either way, Garrus isn't happy. The first time it was challanging, but even on replays I have a tough time deciding. Eventually I came around to a compromise: let Sidonus say his piece, but let Garrus take the shot anyway. lol

Uploading the data from Agent Rawlings was also a tough one, but again, very minor.

Whether to choose Ambassador Udina as councilor or Captain Anderson, I liked Cpt. Anderson more but I felt that there would be consequences for choosing him over Udina in ME3.

Most of the decisions on the Suicide Mission, Bioware did a good job for me of making me really invested in all of my squadmates and I didn't want to lose any of them.

From Mass Effect 1 - Bring Down the Sky and the Rachni queen were both tough for me.....do I kill off an entire species or risk that the queen is lying and comes back to join the reapers for vengence later..........

Another was; to save the Council or not. Of course it is revealed that the Destiny Ascension has a near 10,000 crew (+3 Council), but in terms of tactics it makes more sense (at least to me) to let the Ascension go down and hold back for a full assualt on Sovereign. But I made the Paragon decision in the end.

Virmire was a clear choice for me; Kaiden must die. I never really liked the guy that much, but hey thats me. Plus I'm male so I had both Ashley and Liara.

Mass Effect 2; Garrus and letting him take the shot on Sidonis. I let him take the shot but in hindsight I regretted it. Garrus is messed up by the time you find him, a change I liked as he got rid of that pole as Joker says, but being a fan of Garrus and after everything I went through with him as a Paragon. He learned booger all. Oh how I failed him! :P

Oh that colony one in ME:2, that was heart breaking...And EVIL.

The Genophage. In the end I decided that the best and most honorable thing to do is to let Wrex continue his plans to rebuild the krogan as he planned, so I had Mordin destroy the data.

There weren't any hard choices for me in ME1, and there's were only 2 hard choices in ME2:

1. The Genophage Cure

2. Romance Jack or Miranda

BioWare should really take cues from The Witcher

Oh snap I forgot about the Genophage data. That one really sucked. As janeym27 said, I also made a sandwich before I made that decision. LOL

Deciding which squad members I'm going to kill off on any given playthrough in ME2. I'll choose the unlucky few and then I usually write my list on a note pad. Then I'll plot out which character gets killed off at what point in the suicide run. It's always a difficult decision to make when it comes to picking the character that gets carried off by the swarm. Thankfully I'm sort of methodical in my approach and I always seem to achieve a certain amount of symmetry in my death dealing. All things considered, ME2 is a really fun game to play.

I always tried to stay neutral on the genophage, as I didn't want to anger Mordin. (Always keep the data).

The only one that didn't give me problems, but made me feel bad afterwards was abandoning the workers on Zaeeds loyalty mission. You can hear them scream in agony as you advance through the level. That was a nice touch.

ME 1:

1) Bring Down the Sky. Like the OP said... didn't want to let that Batarian go but then you had to save the hostages.

2) Udina or Anderson for Councilor :|

The only reason this decision was easy to me was if you spared it, it's going into the hands of TIM. I don't trust him whatsoever, so I wouldn't put all that tech into his hands.

Sry for that. I'm in a silly mood.

On a serious note, Probably the hardest decision was probably during Mordin's mission or Garrus'.  I didn't want to let him kill Sidonis, but I didn't really think it was Shepard's call, either

The genophage cure was hard for me too.

The rachni queen was another. I killed her the first time. I wouldn't do that now, though. It felt wrong.
