Me, I only lost Grunt after I chose him to be leader of the distraction team.

I only lost Legion and Grunt on my first playthrough. I'm in the middle of my second playthrough (on Insantiy) and I'll make sure I don't lose a single soul.

Nobody.
To me, the final team selections were really easy, and obvious. But, my boyfriend lost everyone besides Zaeed and Morinth.
I lol'ed. "HARR HAR! You got the (awesome) DLC guy with no dialogue wheel, and space sex-vampire. You are SO f'ed in ME3."

I'm surprised you didn't try to kill every alien on the crew and arrange for Cerberus to get the liquified remnants of your crew to study for reaper tech.

Samara. It's all Miranda's fault. She said she could hold up the barrier, but she failed horribly!
It's funny because earlier she criticized Jacob for volunteering for a job he didn't have the skills to accomplish.

Well here's hoping that Mass Effect 3 will give us the option to allow the alien homeworlds to get purged by the Reapers.

Jacob: Practically a suicide mission. I volunteer.

Samara: Maybe not. I might be able to generate a biotic field to keep them at bay.
Miranda: I could do it too. In theory, any biotic could handle it.
