Can't really see how that can happen when she is behind you. Then again Miranda rarely dies when she is in my party when she is in front for me as well.

thx for all the help guys i have finally completed that mission. I used Garrus/Miranda
Is there anything wrong with Zaeed? I noticed on later missions (get the krogan) he refuses to do what I order him to do and I also noticed he didn't fire his gun at all.

That might be a glitch...
I use Zaheed about as much as garrus, and always miranda. Zaeed is good at stripping armor fast with his frag grenade, and since he has sniper he can fill in that roll... also disrupter ammo does wonders against geth shields
