I modeled my bad karma character after the Joker. Doing VATS mode, when your character clenches their teeth, my Joker has a wide grin as he's hacking people apart with a saw. That's creepy.

Indeed.  And I only play on very hard.

Point Lookout is the greatest b*tch of them all though.  Rednecks with shotguns who hit harder and take more damage than anything short of a Behemoth.
