No, I had her loyal. I took Legion and Garrus to the finale and she was killed during the HTL phase.

I've made both Garrus and Tali fall, while both were in the party. I've noticed that when looking at the heads on the bottom of the screen, the squaddie on the right seems to fall more often than not.
In fact, I've made Tali fall every time by killing myself repeatedly until she appeared on the right.

Between the few quarians we meet in-game, it's either him or Tali to be an admiral. You may as well, this is your project.

I think you're bound for disappointment.  I've caught Garrus instead of Tali and vice versa on otherwise identical runs.  The only constant, I think, is that Miranda is at the top of the priority list if a reload is done during that part.

Then I chose Mordin to escort the crew and had Jack as the biotic specialist. I had Garrus lead the diversion, and I brought Legion and Grunt. And for the finale, I took Garrus and Legion. Miranda turned up dead.

That is all I can tell you.

I was very confused when it happened. My other playthroughs were not so different for the characters I chose for the roles. That's why I think the equation for the HTL phase is not actually set, there may be stray variables.
