Mine was conducted in my scoutmaster's living room when I was like 22.  All the paperwork had been completed and approved and whatnot, but I'd turned 18 and gotten a job and gone to college and never had time to organize an actual CoH.  I didn't even have my medal or cert or anything until one time I bumped into my scoutmaster in a parking lot and he was like, "you need to pick this stuff up."
