So my roommate was playing and in the middle of doing Kal'hirol she left and returned to Vigil's Keep. When she arrived Sigrun was there as if she'd already done the joining. After completeing Kal'hirol she received the quest to have Sigrun perform the joining but could not complete it.
Not sure if it's connected but ever since this happened in her game and every other profile all new party members receive an automatic +100 upon being accepted into the party, rendering fun from building relationships pointless.

Yeah this is one of the major bugs that I and pretty much everyone has been having it seems, sadly it seems that BioWare/EA doesn't seem to think it's an issue worth noting at this time based on the email I received.
