Does anyone have a list on where all the Trainers & Tomes are?

I am planning on making a blood mage but i cant think for the life of me where the blood mage trainer/tome is to do this.

You learn it from the Desire Demon at the end of the Redcliffe questline.

Be sure to enter the Fade with your main character.
