I' ve taken Liara on every missionafte signing her on and mixing my other allies on all subsequent missions. But, I can't get credit for the achievement at game's end. Why?

Liara's achievement is pretty much the hardest of the companion achievements to get. Your best bet is to get off the Citadel as fast as possible without doing (or even picking up) any side missions. Just do what you need to do to get Spectre status, and go get her. Then, go back and do everything, absolutely everything, main missions, and all the side missions, with her in your party. and it should pop when you hit Ilos. You need something like 15 main missions and 45 side missions completed with her in your party. Having Bring Down the Sky helps, as it adds a few more easy side missions.

fred is right, don't do any side missions on the citadel, go get Liara first, then have her with you for the rest of the game and don't go to Ilos before you get the achievement.

Yeah, what the other two have said. Go to Therum as soon as the 3 main planets open up and avoid the Citadel side quests. Extracting minerals also count as well so do find them if you can.

Yup. That's all there is to it. Make recruiting Liara your number one priority; go get her as soon as you leave the Citadel for the first time. Then you can do all the side missions with her in your party for the rest of the game. And the DLC definitely helps; if you have Bring Down the Sky and/or Pinnacle Station, make sure you do them with Liara in your squad. That way her achievement should pop up by the time you travel to Ilos towards the end of the game.

I actually got her achievement on a random side mission before I even got to VIRMIRE. It took me two playthroughs though. Some people claim its glitched, but I don't know. The first time I attempted to get her achievement I didn't get it, and I did 55 side missions(may be exaggerating, but I did more than the "required" minimum amount) with her and the missions I could do with her. Second time when I got it, I only did about 30 side missions with her and it popped up on the Lord Darius side mission. But like the other have said, don't even talk to random people on the Citadel (other than the required ones to achieve Spectre status), and leave for Therum ASAP, and ALWAYS keep her in your party at all times and do as many side missions as possible. DLC missions help

It's because the average Citadel mission doesn't account for a significant percentage of the game, as the majority of the Citadel missions are very brief compared to any one UNC assignment; it only helps if you leave them until after recruting Liara, but ultimately it's not necessary.

it only counts when Liara is there when you complete an assignment, not when you get it. that's what I did. I got all the Citadel assignments, then I got Liara and went back to complete them.
