You can fail it by scaring off Morinth in conversation ("Sooo, how do you feel about justicars?"). Failing it means that Samara isn't loyal.

If you fail a loyalty mission, you don't gain that squaddie's loyalty. When you go through the O-4 relay on the suicide mission, putting that member in charge of anything will get either them or another squad member killed, and increases the odds of them dying at hold the line. You might be able to keep her alive if you keep her with you all the time, then leave her behind for the final battle with Zaeed, Garrus and Grunt holding the line. But replaying is probably the better option.

Ah. Chances are you lost Monrith's intrest. You need to remember what the other characters told you about her likes/dislikes, and keep the conversation going until she invites you back to hers. (the Elcor artist she likes, the gand she wanted tickets to, the Hallix...)

If you've gone too far to go back, remember that for next time. Try and make sure you get everyone else's loyalty, and pick either paragon or renegade and stick with it so you can mediate confronations. If everyone else is loyal you might be able to keep her alive, but I'm not 100% sure...

I failed Thane's the first time. I just reloaded and did it again.

Just out of curiosity, which loyalty missions are possible to fail once you start? I know Samara's and Zaeed's are, and I would assume Thane's (if you don't keep up with the politician) and Tali's (if you go against her wishes during the trial) are as well.

Yeah, you can fail Thane's and Tali's. Tali has a pretty cool dialogue sequence if you fail hers where Shepard says he meant well, but he realizes he screwed up.

And I think that's it, you can't fail any of the others, at least not that I remember.

This has been covered before, but what the hell.

You can only fail Zaeed's, Samara's, Thane's, and Tali's loyalty missions. You fail Zaeed's if you choose to save the workers and don't choose the Paragon option after Vido gets away, you fail Samara's by losing Morinth's interest, you fail Thane's if you don't tell him where that politician Kolyat is tracking is in time, and you fail Tail's if you ignore her request not to present the evidence implicating her father to the Admiralty Board.

It's poseable to fail Tali, Samara and Thanes loyal mission, ironic they are also the only squadmates that you don't need to have.

well, before going to meet her daughter, you need to talk with samara by all conversation option you can have. then she gives you what arak yashi likes like music, drug, her behavior, dangerous stuff.  it is easy.
