In The Fade Mouse got stucked on the hill across where Valor is located and refused to follow me. Had to reload an earlier save.
In the Senior Mage Quarters Duncan refuses to follow me when I am to escort him to his room. It doesn't help to reload an earlier save.
Fortunately I can just walk to the room myself to complete this while Duncan is standing with Irving. I guess he'll be standing there until next chapter.
It's the 3rd time I play through this part, this time after I had reinstalled the game because the new patch killed it. I didn't have this problem the first 2 times.
It's quite common in games that characters don't follow.

Did you turn off the follow option? Just a guess. It's under your portraits, right hand icon, should be a running stick figure rather than a hand.

They're both non-joinable characters so you can't control them like that.
