I believe the New Head Morphs package for IA slightly modifies most of the npcs. Looking at the other picture that you posted of Morrigan it looks like that her makeup and eyebrows might of been changed slightly in the mod.

When I say Morrigan's face is modded, then it's modded. I can't make mistakes when it comes to her

That happens for me whenever i forget to turn on the SQL service. Could be that or wrong database name/password after the reinstall.

On the other hand if you get autogenerated voice just playing the game there might be something more messed up here. At this point might be worth it to uninstall both the toolset and the game, delete the whole Dragon Age folder in Documents (backup the characters obviously) and then install both game and toolset from blank slate.

Did it. Twice infact. I even erased all characters. Then decided to be a [img]http://t1.gstatic.com/images?q=tbn:dE8xaxRraLA4_M:http://ladyfi.files.wordpress.com/2009/01/donkey.jpg[/img] and experiment..
