You have to sleep with Mardy the noble hunter in the Origin to have a son and then you have to tell whoever the new King is to look out for him once you return. And if you return to Orzammar as the head of House Aeducan/Paragon/ender of the Blight who didn't give Harrowmont the Anvil so he dies in a few years and you play your cards right, there's really no reason you couldn't end up King. You stood a good chance in the beginning before the scandal and now you have even more power and popularity.

And with the fertility issues involved with the taint if you want an heir at all, you'd better make sure to seek out Mardy. Or you could always use Bhelen's son as heir but if you end up killing him you're probably not going to want to have to.
