Just throwing this out there I like the game its awesome. The only problem is that when you play the game on insanity mode it is filled with so many bugs. Example using cyro ammo and the enemies freeze yet somehow they still shoot at you. They aren't even free or thawed. Same thing with using biotics throw them up while keeping prefect aim on you and shooting. There are more bugs in there. I don't think they really tested insanity mode at all.

thats never ever happened to me, you may have a localised bugs. never ever had any situation like that and i use cryo and lift a bit

I've never found insanity to have those bugs, it just changes the hit percentages, criticals, etc. to achieve a harder difficulty. But thats just my observations. Still working through insanity though, about 3/4 of the way through.
