Vanguard does seem to get less powerful the higher the difficulty, but its manageable. Teammates are key: you need anyone with Overload/Warp/Incinerate/Concussive Shot to remove shields/armor/barriers at range.
On Insanity, I chose a defensive bonus power (Barrier) and maxed out Charge, Incendiary Ammo and Assault Mastery as soon as possible.
Vanguard also improves quite a bit with the extra weapon specialization, but not for the Claymore (at least for me). I preferred Snipers or Assault Rifles...a carryover from my Infiltrator days, no doubt. Some would probably disagree with me on this, but that's fine with me.
I still prefer the Vanguard to certain other classes on Insanity...

Vanguards have it pretty rough in the early game on the insanity difficulty because they can't use their core strengths (charging and close quarter combat) as effectively, since they lack the powers and upgrades to survive and kill in close quarters. However, once you have Heavy Charge and Champion, as well as a few upgrades, the insanity difficulty becomes fairly easy, once you've mastered when and how to charge.
Usually by the time you get to Horizon, you will have your Charge and Passive Skill maxed out, so don't give up. After that, the class just gets easier when you pick up other talents like Inferno Ammo and Squad Cryo Ammo. Regardless, you have to enjoy the thrill of being on the verge of death to enjoy the vanguard class on insanity. There's nothing more thrilling in this game than surviving on a sliver of health as you continue to charge straight into enemies.

It's pretty tough (or insane) at first... but once you have all the upgrades and skills (right tactics), it will be easier to pull off the Charge skill

After I played my first Insanity playthrough (which was a NG+ Adept) I haven't really had any problems (except for the Collector Ship platforms). I did the collector base no problem.
Just remember to use the right weapon for the right situation, have the right powers and the right squad members.
I did my Vanguard with:
lvl 3 Incendiary ammo
lvl 1 Cryo ammo
lvl 4 Charge (Heavy Charge)
lvl 2 Shockwave
lvl 4 life (Lift Field)
lvl 4 Assualt Mastery (Champion)
lvl 4 Reave (Area Reave)
Make sure that Zaeed and Jack both have Squad ammo maxed. Also make sure that Miranda and Thane both get Unstable Warp maxed asap. Give Samara Area Reave.

So far ive done Soldier, adapt and Vanguard on insanity, and i strangely feel the vanguard was the easiest of the three. The charge shield recharge, and insane shotgun damage just works wonders. You cut down on incomming damage very fast, and you have a ton of mobility to get out of a tough spot quickly
Charging when and what is the most important thing i had to learn, and when i taught myself when/where to charge it became very easy. If its your first time around, or are having a tough spot, dont be afraid to pause the game in tactical view and look around. see where your enemys are, and what kind of cover they are at so you can best estimate your best charge target

darq's right. a vanguard is more than a match for most enemies in the game one-on-one. the only problem lies in knowing when and where to charge. once you start getting a feel for that, vanguard becomes super fun, even on insanity.
in fact, i think i'll be playing my vanguard on insanity AGAIN, now that i've got a better feel for the class.
