How about the Council acting all stubborn, ignorant and everything, even after Shepard proved them wrong about Saren, saved their butts from getting blown up, saved the Citadel, perhaps all life in Galactic civilization...
Both Samara and Jack are of the highest level Biotics and yet their abilities pale compared to Liara, who was essentially an anthropologist.
Why does Shepard gotta make a big show about ordering or not ordering a drink in a bar.
Finally, what's the point of a sex scene if all the characters do is dry hump in their clothes? Couldn't have Bioware just fade to black whenever an ungodly female nipple is about to be exposed?

Not being happy with having an AI on my ship is a Renegade attitude. I guess I should just ignore all the geth, the rogue VI on Luna, the rogue AI on the Citadel and Sovereign.
The Council's complete 180 on their attitude to the Reapers from when ME1 ended. I'd buy them not being able to help because Citadel space is still recovering from the geth invasion, but turning their statement at the end of the first game about acknowledging the Reapers' existence into a complete lie is poor writing.

The stupid C-Sec checkpoint on the Citadel. How can they not know what a Geth looks like? Its been 2 years since the events happened in ME1 and we know that the Geth are seen in various places as told by Anderson.
As for mine:
Having Mordin die, even though I had all the upgrades and made the right choices.
Having a renegade interupt being the only way to damage the gunship.
Seeing Miranda, Jack, or Samara in a hostile, alien evironment with little more than a rebreather.
Telling the Illusive Man to get bent, then going to 'Freedom's Progess' (isn't that ironic) anyway.

I didn't even notice the door on my first play through. Then, when I did see it I had a bit of trouble getting out. Why can't they just have handles like ordinary doors?

In the writers defense the Council was thankful for Sheperd saving them from Saren and his geth. (I played close attention to what they said at the end of the game when I went to do another playthrough in ME 1).

On topic, when I noticed that Miranda's EVA suit was nothing but her normal clothes and a rebreather mask. There needs to be a restraining order against whoever had that bright idea. 200 feet away from anything sci-fi at all times.
