So this bug has happened to me twice now - playing through on insanity, I'll enter the menu and turn squad power usage on or off, and the difficulty has been reset to casual! After seeing this once I started a new playthrough and I check the difficulty at the end of every cutscene, and after talking with the Illusive man, the difficulty got reset to casual again!

Help! anyone know why this is happening? I can't get the achievement if the game keeps resetting the difficulty :(

anyone else seen this? is the difficulty actually being reset, or is the UI just goofing up (does it affect the insanity achievement)?

Yes, it does. You can't change the difficulty at all if you want the achievement. You have to set it to insanity before starting the new game, i think. So, if it's been set to Casual at any point, you might as well start over if that's all you're doing the playthrough for.

Any idea what causes the difficulty to reset?

I started today my first Insanity playthrough and it was going well until the game suddenly felt super easy. I went to check the options menu and difficulty was Casual. I'm 100% sure I set the difficulty to Insanity in the beginning of the game.

I think I did what LEPT0N said but mine didn't reset to casual the text went blank. Also was at the prolog battles at the very start of the game before you get Jacob. Except I had one character on Veteran and the other on Hardcore.

Also to answer LEPT0N's question I belive tragiclly it DOES effect the achivement because it says "You need to finish the game without changeing setting" . Feel free to blame Bioware.
