- Ikutsuki shooting Mitsurugi's father (P3)
- Rush being a remnant himself (Last Remnant)
- Phoenix Wright's characters backgrounds/resolutions (not a complete twist, but they add)
- Guzza being the bad guy (Blade Runner vg by Westwood)
P4's gas attentant was so... insuspicious although the controller vibrated when I shook her hand in the beginning. But at this moment, I already expected something like this. More shocking was Adachi being the villain (or one of the villains). And that such a nice guy (at least he seemed so) was able to do so... awful things. (His voice adds to this.)

That entire game was just one crazy twist after another. You never know who to trust or who's gonna die.

A lot of the haters don't realize CoD does in fact have a story-driven single player. Silly haters.
Really... it wasn't even having Alex die. It was shooting all those civilians and America being invaded that really got to me. Not only was it artfully done, but you SAW the devastation and the consequences of every action. I wouldn't rank the plot twists super-high, but damn was that game emotional.
Nowadays, I still think KotOR still had one of the best plot twists, but Mass Effect's haven't been bad either.

The change in Deus Ex could be seen coming if you kept your eyes and ears open, but it was very well-executed. So yeah, Deus Ex is definitely up there.

Each one in each game was shocking although the one in the third made me think and replay the cutscenes to absorb what happened

Although it did help to clear up the plot greatly

SPOILER ALERT!!!
You spend the game trying to stop an evil mage from destroying a world that's already in pieces as is because of magic abuse, & one of the ones that caused the original destruction is... um... Doh! I forget his name.
Can you imagine the irony? Start life as an evil mage, triumph over an older wiser good mage, get foiled by the hero main character, escape into a magic portal, wake up & realize how stupid evil was, turn good, work toward the destruction of your own younger evil self, then eventually die at the hands of your own younger, stupider, evil self?
I can't speak for anyone but myself, but I think this was one of the coolest plot twists ever!
END OF SPOILERS!!!
(I hope I did the Spoiler Alert correctly. There is no freaking way I coulda described this properly in a 'beat-around-the-bush' kinda way. I'm a fan of that strategy, but unfortunately, it's not always managable. Even spelled out, I hope I didn't screw up at describing it correctly as is.)

Because the virus led to the artificial release of the decoded areas in the human genome.
