That ending would be pretty damn nihilistic. I mean, it sounds pretty cool and would be totally ballsy of BioWare to do something like that, but I doubt it would happen.
Whether Shepard lives or dies in ME3, the victory should feel very hard. We're talking species going extinct, crew members getting killed off - the Reapers really need to feel like they are a terrifying force. If everybody lives at the end and nothing is sacrificed, the victory is going to feel so cheap and hollow. Some folks are going to have to die, no matter what. No reloads, no saves, no metagaming. They just kick the bucket and you have to deal with it.

I hope there is not going to be any happily ever after ending in ME3, that just would not feel right.
As for Shepard, I think killing him/her would be good for the franchise - no matter how good-designed and well-written the future PCs may be, people would want more Shepard instead.

I want Shepard to live and be happy with Garrus and go fight crime on Omega...And then adopt and raise children and live happily ever after... But happy endings are so far and few between

Neither any "all crew members and friends and their grandparents should die" nonsense either. Well prepared team who's working in coordinated and in intelligent way will be OK. Sure, Reapers may wipe out some star systems and such but so what. If they really want it, then they can add an option there. Like option A, the "stupid hero option" where he chest pummels himself and then goes and gets himself killed, "for the sake of all". Option B, the "wise survivor way", where he instead of "standing in the way of the lethal blast to take one for the Galaxy" will instead disable the gun. Less bravado, longer life span. And then the option C where he could say "Hey person_next_to_Shepard_01, was nice to have you along for the run but now it's time for you to save the Galaxy" and then pushes that person in front of the blast, so to say. Bling, plenty of Renegade points and the Galaxy saved too.
In any case, those stupid "take one for everyone else I really don't care about" kind of self sacrifices or "kill all you know in the game of greater good" nonsense is stupid and should not be forced on player, if they're in there. Add seveal choices as such, but not any default forced on ones. I liked the end options, for example, in Fallout 3 - why on Earth would my char be forced do go and get himself irradiated to death (well, before the Broken Steel DLC anyway) for some Wasteland he doesn't care that much about? He did a good thing anyway already by finding out Enclaves plans, dealing with them and stopping their actions, quite enough already. And then go and kill himself for random people as well? What the hell? He didn't survive all that just to die like some schmuck. If that other BoS member wanted to do it, heck, go ahead and "take one for the world", I've done plenty enough and the world already owes plenty to me, I don't owe anything to the world. Same in ME series, whole Galaxy already owes me more then they could ever compensate, I don't owe them anything and won't "take one" for them either.

He'll be a leader of the new Galactic Navy, which will be comprised of races across all the galaxy, rather than just "Turian" "Human" "Quarian" "Geth" fleets.
