While I'm pretty sure that BioWare already have their own ideas brewing, I'd be tempted to write Xen in as the Saren equivalent in ME3. There's some heavy foreshadowing on her character during Tali's loyalty quest and Ascension outright indicates there are quarians who are seriously considering using a Reaper to regain control of the geth. It also works in a poetic fashion, because there's an ironic narrative to it - in ME1, the geth were aligned against you by Saren and Sovereign. In ME3, a rogue faction (I doubt most quarians would align with Xen) of quarians led by Xen move against you with the Reapers.
There's a part of me that's just tickled by the idea. I could see a Spectre sent by the Council to take you down (given that your activities must look very suspicious in their eyes) could work as well, though.

An interesting idea Nivenus. There was some definite foreshadowing. I'm not sure that Xen would be the main villain though. Her plan to control the Geth seems to be doomed to fail for any successes come from it.
The Spectre idea is a nice one. The Council could declare Shepard rogue or a danger to the galaxy, maybe even putting Shepard on trial.
