I decided on this playthrough...my tenth game...that I would try different choices for my suicide misson and used Kasumi for going through the vents and as she was closing the door ...she got shot in the head! Then I used Miranda to protect the team with her biotic fields but Jack got carried away by the seeker swarms, screaming!! That was a haunting death! And of course Miranda dies at the end since she got angry at Shep for siding with Jack.
So at the end it was just Shep and Samara running for the ship! I noticed that after the misson if I chose to save the base that background behind TIM is red and if I chose to destroy the base the background is a calm blue...perhaps an indication of the state of the universe.
