For some reason, the Return to Ostagar quest isn't considered completed for me. I have all of Cailan's equipment, both of Duncan's swords and gave Cailan a proper funeral, but the quest still acts as if there are still equipment pieces left.
Help?

I can't remember: do you find the letters to/from the Orlesian empress in the armor chest? If not, that could be what's keeping the quest open.

Yeah, I got the documents (and the sword). The journal does acknowledge that I completed the "find the documents" part of the quest, but not the armor part, for some reason.
Also, this may or may not be related, but Alistair and Wynne commented as if I had the full set...after only getting 3 pieces. Wierd.

Well, sounds like you discovered a bug. Yay you! Unfortunately, the only easy fix would be to revert to an earlier save and re-do the armor collecting and hope the game doesn't get confused about what has and hasn't been done again.

Encountered the same problem and even though this thread is old, I'll post my solution just in case someone needs it in the future.
The game counts every piece of Cailan's equipment as two, as can be seen in journal. There are 6 pieces of equipment in total. After picking up any piece, the journal says: 4 more to go, then 2 more to go and it updates the collecting part of quest as completed with collection of the third piece. At the end of the quest, however, the game expects player to have 6 pieces in his inventory, but since it counts every part for two, it sees the number of items as 12 and gets confused.
I bypassed this bug by not collecting Cailan's equipment from the start. I made my way through darkspawn all the way to the reanimated ogre, I looted the helmet from the necromancer and returned to bury Cailan. Only after that I went to collect the documents and after that, the rest of lost equipment. Quest got marked as completed after collection of the third piece, but you can still collect the rest without breaking the quest.
