For starters, the many goofs of giving an assault rifle to someone who can't equip it:

Although I don't have screenshots of it, I've also seen Thane and Jack carrying this AR.

At this point, Legion had been equipped with a Geth Pulse Rifle. So what is this he's holding?
It seems they're all using some generic AR, although I don't know which one it is.
And some pistol goofs:

This one is particularly strange, because there's no reason for Legion to be armed and he needs both his hands free to hack the doors. It also appeared when he was hacking the Derelict Reaper computers.

And another goof for Legion, who seems to be a frequent victim:

I don't know which sniper rifle this is, but it's clearly not his Widow Anti-Material sniper rifle.
Now for the one that really annoys me, Shepherd:

This is the only time Shepherd had my preferred weapon out in a cutscene.

The rest of the time, it was this one:
Considering the amount of customization they did so Shepherd would have a unique appearance in cutscenes, why didn't they do something similar for weapons?
For example, adding a code that flagged a weapon as a "primary weapon" so that it would always be used during cutscenes and during gameplay.
I always found it annoying for Thane and Tali to randomly switch their weapons after a savegame load or a cutscene.
For example, Thane once had a sniper rifle out for close quarters combat.
And Tali always seems to equip shotgun, no matter how many times I equip pistol on her.
