The wiki makes no mention of where it is acquired.
The official Prima guide says it is sold by the New Shop Keeper in Haven, which I have never encountered. The "old" shop keeper does not have it, even at a point when other merchants have ther "2nd store" inventories available. I only wonder if there is a condition where the shop keeper in Haven lives, and after the Urn quest is complete, perhaps then the inventory refreshes? I dunno.
Anyway, does anyone know where to find this recipe?

Wish I could change this topic title to "Haven Shop Keeper" as it seems he is the issue.
In the Prima guide, he has two stocks of inventory (like most of the major merchants in the game). In his second stock, he offers a Tome of the Mortal Vessel and the recipe from my initial post.
But I just tested this out and he does not spawn any new items like the other merchants do (you know, after you've completed some of the main blight quests).
He is not even listed as a merchant on the DA wiki, and there is no mention of his Tome. I'm assuming he is either bugged or his 2nd store inventory was intentionally removed from the game.
Can anyone confirm or deny this?

His second inventory never pops. Once the shopkeeper is killed, along with all the other cultists, a replacement does not respawn. If you do Haven fourth or fifth, his inventory does not update. The information in the Prima guide is wrong.
