I just finished the game recently with an Adapt on Hardcore, a soldier on Veteran, and another adapt but on Veteran. I encountered ALOT of problems on each of my playthroughs...

Adapt (Hardcore)
1: Difficulty setting text went blank= I started a male engineer and fast forwarded some diolog or somthing or had a conversation with someone then clicked on my difficulty setting. for some reason the text in the difficulty setting went blank. I switched to my adapt on hardcore who was Horizon and noticed I had the same problem. I exited the game and returned and problem was fixed.

2: Ally positioning on Collector ship: first fight= They werent covering the way they should. Not sure if this is a serious problem but a minor.

3: No safe place to cover on Collector ship: first fight= Kept getting killed due to Harbingers or collectors attacks near me while hideing in cover. had to lower difficulty. Was upset but I think this is a minor problem. 

4: Cover glitch on Get Thane Mission= Theres a part with a wide hallway and only two robots come toward you shortly before the dropship fight. if I hide and shoot from a back wall, I fly up the wall and get stuck on the wall premanantly nad have to reload game.

5: Samara VS Morinth 1: No paragon/Renegade choice to be able to pick= During the part where Morinth says "tell me you want me, tell me you will kill for me" I am forced to choose the middle choice. My paragon meter is 3 and 1/2 full from a full bar and renegade is 2 bars at this point. I ask myself why?

6: Samara VS Morinth 2: No audio= While there shooting the energy beams at each other the audio cuts out, the sound comes back in when Morinth is crawling on the ground.

7: Disable False signal no audio= I accidently clicked on the datapad near the door a second time and had to play the whole mission with no sound, When I realized it seemed permanant, I was going to get back to my Xboxlive screen, when the second chime kicked in, I chose to resume game. Sound returned.

8: Jack not loyal due to paragon/renegade choice in grey= similar to my situation with Samara and Morinth. My paragon bar is 3 and 1/2 and renegade at 2 bars. I chose to make Jack mad. I did a few more missions and made my paragon on the last bar but not full and had no way of makeing her happy. I still ask myself was it because I spent more time doing the missions and the Firewalker DLC whish is why there was no way to make Jack Loyal?

9: Cover quick teleportation= During the final battle I was being attacked by a Harbinger. I was hideing behind a good size and far away cover then I found myself magically teleported in front of a Harbinger and was like ><. I got lucky and got him but found that really baaaaaaad.

10: Normandy crash site 1: Return to Mission= I can return to the mission anytime I want.

11: Normandy crash site 2: Placeing monument sends you to outerspace literally= When i return to the mission I can place the monument again. When I do so I float to outerspace and die instantly.


Soldier: (Veteran)

Adapt (Veteran)

1: Normandy crash site 1: Return to Mission= I can return to the mission anytime I want. I chose not to since I know what would happen if I do.

I usually have occasionly game freezes on some points but it seems to be random but I also seem to be stuck in the one room on occasion at the end of Mordin's loyalty mission but it works and sometimes doesn't work. However I am primarly really really really mad at the fact that I had these issues since the Kasumi Goto and new patch came out because now the game seems like a very fustrating game to play because it's already bad enough that I have router issues which tends to make the game unplayable due to the fact that I downloaded a few things in the game and need my account on when the title screen appears. But with these new glitches, bugs, cover issues with characters, and unusual ways to die or get permanantly stuck makes the game seem very unplayable regardless of difficulty. All I can say is please fix....PLEASE.
