That an easy fix.
Just have more levels with low atmosphere environments so you have to wear a helmet all the time or die.

They could just leave that feature out for masks with speech bonuses.

The only potential problem I could see with this is the hair. With a helmet like the recon hood that is very tight on the head, the longer hair styles would clip through the helmet. This doesn't happen with the current game because it would seem there is no hair rendered with the helmets on. They'd have a few options to get around this.
1. Make every hair style very short. Easiest option, but kills character customization.
2. Somehow have the hair disappear as the helmet forms around the head. I'm assuming this would be a very complicated animation process and just not worth Bioware's time.
3. Make every helmet big enough to completely cover all hair styles. This would limit them as to the variety of helmet styles they could create.
Unless someone has a better idea as to how they could accomplish this, I fear that it just won't happen.

Maybe if they took time on developing ME3 they could utilize it.
