While I think its a fairly large oversight on the developers part, the quest givers tell you once you start one side of these quests, the other faction will no longer speak with you. So in future it might be best to listen or read the entire dialog instead of skipping it and just reading the quest log.
Orzammar main quest will not let me advance. NEED MAJOR HELP!

This bug is kind of odd, really it should force you into a decision, so that you have to pick one or the other. The deal breaker for me was when I went through Harrowmount's house and found a scroll that the king had wrote to Harrowmount in which he had reservations about Bhelen. Of course Harrowmount could have planted that scroll there for me to read.......decisions, decisions!

I can not say this enough. Always have multiple saves. When in doubt, create a new save.

Hrm, well I started off supporting Harrowmont, got all his quests (but didn't complete them). Then I went and talekd to Bhelen's flunky, and he gave me quests, but specifically warned me that if I did the proving fight for harrowmont, all deals were off. So I can imagine if I did the initial quests for Bhelen (thus pissing Harrowmont off), and then went and did the fight, I would also have pissed Harrowmont off, thus cutting myself off from both. You get warned, I don't think this is necessarily a bug.

People need to stop saying this isn't a bug. Its very annoying since you don't understand what has happened. The game does not register that you fought and won the proving for Harrowmont. Your quest log says that you failed to represent Harrowmont in the proving even if you fought for him and won. If you go to the Royal Palace, the guard tells to go right in and talk to Bhelen, but the door wont open. You have no dialog options to fix this or pick a side.
Any number of things could fix this. The simplest being that you just open the damn door in the Royal Palace. At least make it possible to pick the lock. Then we could talk to Bhelen and report to Harrowmont's representative and finish the game. Other solutions could include the ability to tell Dice and the other guy that the documents were fake.
What bothers me about this is that it is literally a game breaking bug. I didn't think those existed on consoles anymore. I can deal with everything else, the dex bug, spells and talents not being accurately described, spells not working as intended (did you know according to the wiki disorient actually does nothing), over powered bs that is clearly not working as intended (see shimmering shield), and dumb mob pathing bugs (the people soloing the high dragon are not using the terrain to their advantage, they are exploiting a pathing bug).
If you pay 60 dollars for a game you need to be able to finish it out of the box. The existence of a game ending bug should be a much bigger deal for Bioware. Its very disturbing that it does not seem like it matters to them. I have to seriously reconsider purchasing Mass Effect 2 now.

Same problem here. In the beginning I chose to do quests for Bhelen, but after all I gave the Crown to Harrowmont and the quest "A Paragon of Her Kind" doesn't complete. It says that I must return to Orzammar to crown the king.
And there's no way back, my autosaves are after leaving this place, and my save is right after the crowning...

I, luckily, create a new save about two or three times per main quest, sometimes more, so I always have the option of reloading an earlier save and only losing an hour or two.
For the whole A Paragon of Her Kind thing, though, I decided to try out the Orzammar debug menu through the developer console instead. Not an option for 360/PS3 players, however, so it's not a legitimate alternative to fixing the quest.
