I have beaten the game three times now, once on Hardcore (I have the achievement to prove it). Now I want to tackle Insanity, but when I try and start a new game, Insanity is not unlocked. Weirder still is that Hardcore is not unlocked. Is there something obvious I am missing? Thanks!

I'm guessing Mass Effect's own data got reset, whilst Microsoft continues to track your achievements correctly.
I'm not honestly sure how to resolve this on XBox. It's possible you just need to complete the game again (which shouldn't take too long to try if you still have a save), but it's possible that already having the achievement in Microsoft's system could mess things up.

I am having the same problem but with unlocking hardcore. Beat the game 4 times and no Luck. Did you resolve this?

I'd say it's probably worth a try for you, assuming you too still have a save just before the end of the game. People are more likely to report back if the solution didn't work.

Yea, i have tried multiple times. Looks like I'm screwed. Only thing I can think of is deleting all my saves and starting over. But not before I get the 60th lvl and the rest of the biotic ach. Another weird thing was the asari ach. I used liara since I got her and still no ach. Ahhhh

You have to go and get her right away and dont do any of the citadel side quests until you have her as far as i remember

The message under hardcore and insanity is misleading.
It always says it needs to be unlocked even when it is unlocked.
So just ignore the message about needing to unlock insanity. You should still be able to select insanity. Just make sure you select the save settings option afterwards.
