This has been happening to me quite frequently in my Insanity playthrough (Only happened once), where my Shepard gets stuck facing one position, yet the camera can still pan around him. What this ends up doing is that in combat, although I can still shoot the enemy, Shepards body covers the entire screen. As far as I know, the only way of getting out is by pressing menu, going into cover, or switching weapons. They don't always work however.
