I named my female warrior Gardenia because I thinks it's a pretty name and because Gardenias are my favorite flower.

City Elf Trisanna, who is a nod to Tamora Pierce's Circle of Magic books in both name and appearance, though said character was actually a mage.
Human Noble Vlad, who is modeled after Vlad the Impaler.
City Elf Mouse, who has no reasoning behind his name other than I thought it would be funny to try to make him look like a mouse and name his Mabari Cat.
Dwarf Commoner Memphis, because I have a friend who lives there.
Dwarf Noble Tamriel, because I was on an Oblivion kick at the time.
Elf Mage Sever, who I had intended to name Severus as a nod to Harry Potter, but Sever suited him as a character better.
Human Mage Reynard, who is in fact a female and was named because I like foxes.
Human Orlesian Warden Belle, as a nod to Beauty and the Beast.
Dalish Elf Sirius, also a HP reference.
Dalish Elf Devon, named for Disturbed's song "Inside the Fire".
Human Noble Wren, I just happened to like the name.

For quite a while, I named my characters after characters from Roger Zelazny's Book of Amber series. So my first character was a human mage named Mandor, my second was a human noble named Cain, and my third was a dwarven noble named Oberon.
My fourth character, however, is named Lionel, just because I like the sound of "Lionel Amell."

I only tried out the intro story for the other races; dawrf and dalish just to see the difference. Other than that, all of my completed playthroughs are human nobles. I did try the mage story too but did not completed it. Why only human nobles? After the first completed run, I had the inkling that the human noble has the most influence within the structure of the story and beyond. If the fabric of the story were focused on the dwarves / Orzamar / Deep Roads, I'd stick with a dwarf and so on.
In any case, I named a few canon nobles thus so :
Pakse - after the Paladin in EM's novels on Paksenarrion or Sheepfarmer's Daughter
Lelix - no particular reference to any char, the first letter is just a reminder of which char is LI
Karrigan - after the main char of CB's Green Rider novels
Aedelian - nobody in mind, just a guy Warden who married Anora
I once read a guide on romances and the author noted that Leliana always came on to guy Wardens. The funny thing was, she always seemed to be the one to take the first step with all my female Wardens. I played no differently with the guy Warden, always almost the same response but she never flirted.
I have not tried Morrigan yet, she seems daunting.

Angus, because it's an Irish name as is consistent with Fereldan naming conventions. It also plays nicely off of Fergus' name, which seems quite fitting considering he's the HN's brother.

1st DAO char was an elf mage Traiklaeme... awful name intended as "Trai" because I was just trying DAO out. The "klaeme" part was just intuitively making a sound to offset "Trai" and also like "staking a claim..." Had to end the char on the basis of the name alone.
2nd was a male dwarf commoner rogue named Akgrogan- a name I grafted from a dwarf fighter/ thief I'd made a year ago for a no-reload Baldur's Gate 1 game elsewhere in these forums (fora). He didn't last long either... The name itself came from an attempt to make a name that lent itself to a Scottish accent (which is the accent of so many dwarves in BG1), and I still like simply stating "Akgrogan" out loud (hehe), but the DAO char build and look for him was so awful, and the char story and personality so unlike the original BG Akgrogan, I deleted him by Ostagar.
Not sure of order after that but that 2nd char introduced me to how sensational dual wielding dwarf girls look, so I started the first Kruklia, another dwarf commoner rogue. She's gone through several attempts at a playable rogue build and game until this final one spelled Kruklya instead which I like better. The char name originated from NWN1 in which my 1st char ever was a dwarf girl fighter/ rogue named Kruklia (with an "i") Battleforge. The name Kruklya sounds and looks almost Polish but was intended as just a play on the word "crook" given that the char was a thief... Plus I like the letter "K" a lot.
I also early on made a dwarf noble shield warrior named Jagan who also began outside DAO as a dwarf fighter or cleric (or both) in a number of player-made NWN1 servers online. Like most names I've come up with it was mostly just from bouncing sounds in my mind and trying out consonants (in this case "J"), but in this case the name also turned out fairly simple- unlike the more multisyllabic names I usually generate- and since I've not seen the name used anywhere else, and I've never developed the character particularly far anywhere I've tried to start him despite liking him, I've kept him in storage a while. At present- planning a return for him later as a 2-handed weapon warrior with a more wizened complexion... He's looked far too baby-faced before... and again the build was awful...
Then I got curious about the city elf and tried that with a male rogue- Aedrynel- but he's been deleted and resurrected in slightly different forms (and names) a number of times now. He presently has a sister who languishes in my list (planning to run her again differently also) named Aedlyn. Her new name was planned as Aedylnae, but it will stay Aedlyn. So obviously the family all has the "Ae" start, a vowel combination I tend to reserve for elves- though the Aeducans would object. Given the city elf backstory I wanted something less ostentatiously elven (since humans would probably have dulled elf names down) but yet I still wanted one that retained a sylvan core... Even though DAO is hardly the best game for exploring elf identiy...
Since the Tower mage elf origin has no definitive backstory specifically as an elf I made a female Traiklaeme replacement with the more elfy name Sywynlae. That name was the product of 1. recognizing that i'd never made a single name starting in "S" and accepting the challenge, and 2. trying to make it sound elven. Thus I injected yet more "ae" and spamming the "y", but it works ok. Sywynlae has gotten the furthest of any of my chars- just before Landsmeet- followed now by the wonderfully successful Kruklya.
Another name to enter was Donner, another char from NWN1, this one intended for use as an intelligent human docks fighter-rogue thug on a server where I was briefly a DM, but after that venue died, the char resurfaced as my own player char on another NWN1 server (same one as Jagan) as a sort of harder edge alterego. The name was intended like Jagan to be simple, but in Donner's case it was coupled with a more pretentious last name- something like Carrio-Entrizzi- due to a character story of having had important or wealthy great forebears or relatives in the too-distant past- with Donner born in the slums. It seemed to work with the human DAO origin story a bit due to actually being born into royalty and cast out, but his NWN1 tendencies are a bit too gritty for the cozy family story of DAO lordship, so he's only being retained as a portrait face remotely similar to my real one in case I lose my DAO dwarf appreciation at some point. Not sure how I'll find a way to play a Cousland all the way through...
As a side jest, someone (I believe from Britain) once ribbed me about Donner being a "girl's name," insisting on it repeatedly, and only much later did I recognize he was confusing Donner with Donna... The name itself has no conscious architecture- not intended as "one who dons things" or "Don Juan" or anything- just shifted some phonetics about and out came "Donner," sounding plain and apt for a human.
[EDIT: Forgot to mention my Dalish elf archer rogue attempt that got deleted around Lothering- Feylathil- yet another name from the same NWN1 server in which I produced Jagan. Although the "fey" first syllable appears an overt reference, I struggled with the name a lot before that first syllable appeared as such: Fellathyl, Fe'lathil, (Falafel?), etc., until I settled on that. It was another phonetics jumble, going for elfiness but least this time without the "ae" for once!]
My last languishing char is Blunchard the dwarf noble rogue. I was trying for a name that was somehow comical and cool at the same time. Makes me smile to pronounce it. "Why, hello, my name is Blunchard." It was inspired in part- aesthetically anyway- by the real life murdered Black Panther Party leader named Bunchie Carter... with such a comical name for such a tough and tragic figure. This also vaguely fits Blunchard's own kindly approach to Aeducanhood met by a cruel betrayal and a brush with death.
[EDIT: For Dog names I've not been particularly choosy since I'm not a big fan of dogs or of that party addition. First it was Mabari which led to amusing confusion in combat with other mabari. Then I went with conversation cues. When first encountering it, Morrigan calls it a Mangy Beast, so it's been Mangy, Beast, and Mangy Beast. Morrigan also says she hopes it's more useful than Alistair, so I called it Useful. Alistair counters Morrigan with some other adjective I ended up using. Kruklya's mabari is named Krak just because I ran out of conversation cues after so many times and it sounded like a good name for hers... but languishes at the camp like all the mabaris have so far... If I run a human through I'll have to try keeping the mabari...]

First PC I played, I just used the default name; had NO clue what I was doing and literally limped through the game.....it was hilarious. So after that first horrendously messy play through, I examined what I had learned, and read up a bit on the internet (mostly gamefaqs), and eventually played a number of different PC's.
1. Female HN, Neema - I swear she was not named after Tali in ME.....I had never even looked at that game at the time I created this PC. Still, it is funny. I just thought it was a nice name.
2. Female Dalish Elf, Zyla - I was really in to naming characters that started with the letter "z". Just because.....
3. Female City Elf, Tevara - Again, not sure what prompted the name, but thought it sounded "elfy".
4. Female Amell Mage, Raven - Since I made her after DA2 came out, and knew that she would be the cousin to Hawke, I chose a bird name.

6. Female Dwarf Commoner, Jaxis - I new a woman named "Jax" years ago at an Internet company I used to work at. Her name just struck me, thats all.
7. Male HN Hayden - I had just finished watching "Sideways" right before creating this PC. Took the Hayden (yes, I spell it differently) out of Thomas Haden Church. It seemed fitting, since he is my d-bag warden. Not evil; just a douche.

The City Elf is a hereditary servant, who might become a butler or something similarly respectably if he is extraordinarily lucky and hardworking. So, naturally, I named my CE the name that sets him up to be a butler, and then made him an assassin just to be ironic.

I go with the default names for most of my PCs, though I haven't finished a play through with any of them but the HN Female.
But I do have some names of my own.
Female HN - Illyria, after the character from Angel
Female HM - Talia, after a character from Jim C. Hines Princess Series, which are strange interpretations of fairytales.
Male HM - Emery, because I bugged my roommate for a name and that's the one she came up with.
Female EM - Allyn, from a name I found in the Sims 2
Female DE - Aria, because a family friend had a dog named Aria
