Quite to the contrary, you get a new side quest with Mardy, if you don't use one.

It's not explained in the game if there is a method of birth control.Maybe they use herbs?

Maybe they have spells that can cure STD's?
Although in Awakenings, Oghren complains to the PC that he has a rash on his crotch and he wants to look for some minty balm.Since dwarves are immune to magic, a magic cure wouldn't work on them.
