To some people, Shepard's private bathroom.
I am one of those people, when I first saw it I nearly crapped my pants.
I hope you don't always crap your pants when you see a bathroom. It's messy.
But the consort was one of my WTF moments. The look on the face of my brother was priceless.

Hell yes.
Had been going through with Jacob thinking..this is pretty rubbish, hope someone else comes along soon. Then casually talking with Garrus and those hints appear and its a mad dash to run back to Jacob saying "get lost". Easily going from the worst romance to the best

No glitch that I know of. Sometimes the game just assumed you made a certain choice when you really made a different one.

Oh, and I forgot something too: When Garrus got shot in the stomach during the suicide mission (of course, it's not just him it happens to)...I was like, NOOOO! Then he shakes it off and keeps on going. Phew! XD
That freaked me out the first time I saw that...
I was like, "NO!!!! I just strip mined half the !@#$'ing galaxy to get the upgrades! I did every god!@#$ loyalty mission! I put everyone in the right job!!!!"
And then he just shrugs it off...
That was quite evil of BW... I loved it...

Probably.
Or Conrad is just remembering what he wants to remember. His relationship with reality is tenuous...

Hell, YEAH! I went "NOOOOOOOOOOOOO! " when I saw it for the first time. My roommate came in and yelled "What ME mission did you mess up this time?". Seems I'm obsessed....or something.

The quest is glitched, it tags you as having both charmed and intimidated him, and apparently the intimidate takes priority. It's not all bad though, you can get a discount in that weapons shop on Illium by talking to him and charming all around.
As for my own wtf moment, has to have been the bathroom XD
I can't remember what I was doing but that panel suddenly sliding back and revealing a working toilet was very unexpected. I find it odd how close you have to get to it for open, probably why most miss it

ME2: Bathrooms... also... my toilet flushes...
Lack of dialog... oh you have to calibrate? I'll just come back in a few missions and you'll have something to say... but he doesn't wtf
Being recognized with a helmet on
Drinking with a helmet on
Passing out from getting drunk (great behavior for the savior of the galaxy {paragon})
Dead fish
Kelly (Just everything about her... I like her but... she always has something interesting to say... unlike everyone else)
Playing as Joker
Killing Joker (Not me, was watching some one play)
Jacob: telling me the engine is "lighting up like a Christmas tree"
And a positive note to end on: The Omega 4 relay. The view after coming through it was really awesome as was the fight with the collector ship.
Sorry this list is so long... I was pretty much like this

When I first met Zaeed I thought "Damn, your an ugly b****** aren't ya?" but Zaeed doesn't need good looks to kick ass.
Also I wasn't expecting to meet Tali or any member of the crew as early as Freedom's Progress. I hadn't played ME1 in a long time so I was thinking "I remember you... kinda."
And meeting Wrex wasn't quite a WTF moment, but the way he gets up and says "Shepard!" was touching to even a cold-hearted SOB like myself.

EDI: Shepard, the real reason the Collectors have been abducting colonies through the whole game is to turn humans into slurpees so they could make a T-800 Reaper baby out of the human slurpee goo.
Wtf.
And not a good wtf, either. Not a "omg I have a bathroom wtf!"

Oh forgot to mention Biotic charging a Collector off a platform to fall to its death only to hear "Assuming Direct Control" a few seconds later XD
Had something similar, only it was a collector that had just been flung into the air and over the edge by a shockwave.
"Assuming Direct Control"
"...."
"Releasing control of this form."
Edit: typo
