With Precision Striking activated, I found myself landing most of my hits. I do miss alot, but that was simply because I was trying to achieve 38 str as soon as possible for the Dragon Armor.
You will need Dex if you want to have reliable damage however.

Actually it does rather well, i never put any dex into my charector the first playthrough at all. Only time i really missed a lot in a row was when i had misdirection hex placed on me.

Get both dex and str, both have their uses, and yes, misdirection hex can be fatal for a warrior(but so fun! to use on a melee boss

Strength adds just as much Attack as Dexterity (0.5 each). Unless you're using a weapon which depends on dexterity (which you don't) you should safely be able to ignore dexterity.
You'll want some Willpower to go with that Str/Con, though.

I've done my second 2hander playthrough, and let me give you the gist. For the warrior, dex does not increase accuracy, but it increases your ability to dodge and parry attacks. For my first playthrough, I had high strength, moderate dex. For my second playthrough, I went all strength (you can look at the stats of this character in my profile). It appears that there were no significant differences in the ability to hit the target. The greater strength did however, allow much higher DPS. Using dex seemed like a waste for the first character. Although in theory he had an increased ability to dodge attacks, it seemed worthless given that I had a tank to absorb most of the attacks.
