For me, no matter how renegade I am I can never kill Shiala. She's just obviously on your side and trying to help you, so I can't kill her on tactical grounds, and even if she weren't cooperative she could have valuable intelligence. All the bad things she did were under indoctrination so I can't kill her on moral grounds as she wasn't in control of herself. There's no justification for it, no matter how ends justify the means you are or how paranoid you are. Unless you're just a recklessly killing her out of spite, ignoring how useful she could be. It just seems willfully ignorant and I can never do it.
I can also never, even on the most paragon-y of paragons, defend the Illusive man to my squadmates on the collector ship. I don't care how forgiving or willing to see the good side of people you are, I cannot play idiot Shepard. Just pisses me off too much.
Similarly, I feel like shooting Marvin in the face is something the game pushes you into way too much. It's really really tempting, and hard not to do when Jules drives the car over that bump, but it's really senseless. He's not that bad of a guy and on top of that it doesn't serve any useful purpose to the plot but force you to do all that cover up BS. I have no idea why anybody would choose to do this.
But those are just me. What are the decisions you guys can never seem to do in any of your playthroughs?

(no particular order)
a. Not doing Miranda's loyalty mission
b. Giving the evidence at Tali's trial
c. Saving the collector base
d. Not doing Samara's loyalty mission
e. Saving Morinth
I can't remember any other ones. Those 5 came to mind. They just seem too cruel or not right.

I admit I'm not much of a roleplayer. All my characters seem to work from a place of logic.
But, there are several things I just can't do either - I can't defend TIM (I question why it's paragon to do so). I can't not shoot the gas tank to set the krogan on fire. Or are we talking major decisions?

Of course, but then I could have sleep in my bed hole time and avoid all killing.
It's a question between someone attacking you and you doing cruel thing and execute someone.
Also it's the different between what is necassarity for you mission and what's not.
If I example not execute "Fist", what different does it make for my mission, do I it or not.. If I how ever do or not squad members loyalty mission, how would I know how it's affecting squad members performance while critical mission situation.

For my sake, Mr. Taylor has never left that planet alive, no matter how much a given Shepard values life, human life, or even the law. I imagine they've felt bad aboard the Kodiak, and possibly been sick aboard the Normandy over it, but right there on scene, the glassy stares and fumbling, frightened words of his camp women dancing in our eyes and ringing in our ears, disgust always wins out and he gets no more consideration than a leech stuck to your foot. The kind Shepards have him shot - the rest leave him to his mess.
I've never had a Shepard who was hard enough to pull the genocide switch. Zhu's Hope is a graveyard, the Council is doomed, and everybody that's given me a problem and the option has been shot in the face: Fist, Ethan Jeong, Marvin, Finch, Balak...
But somehow, the extermination of a unique, sapient species just seems a step too far. The Thorian never gives you the option, but the Queen...not once.
Which makes me wonder: is the acid bath as viscerally awful a scene as shooting Wrex was? I felt a little nauseous the first time, and every time since (done purely for the Renegade points on 1st playthroughs of level 60 import characters) I've had to mute it before I select the option.

I dont get why is paragon renegade to catch somebody off guard.
Is parago suppose to be "fight with a moral compass but with a military mind" not "fight 18th century style of honor at the cost of somebody blasting you witha shotgun"
If the dude is speaking is just efficient to catch him off guard....never understood why is a "renegade" option to shoot the tank....
or IN miri's recruitment to break the guys neck off and the like.<_<

Because a paragon refuses to resort to violence until it's clear that the opponent is unwilling to negotiate. Compared to renegade, who doesn't care and takes the first opening he gets.

Likewise, Miranda's mission where the guy says he's had snipers lining up their shots...it just seems too stupid to allow them to attack first after that. You've tried reasoning with the Eclipse and now they're telling you they have snipers on you.
My Renegades cannot ever turn the Collector Base over to Cerberus. That's a huge mistake. The Illusive Man does not have the wisdom, intelligence, or cunning to use that base....I doubt anyone does. Any knowledge gained from keeping the base is likely already gotten from EDI since it's clear that the Base has no defenses they wouldn't expect an AI to hack their systems (in fact, she gets info rather quickly on the Reaper Larva due to their lack of cyber defense).
Also, I gotta hug Tali. Her dad just died on the day that she was accused of treason. It's a rough day.

Well, from my view....you can tell who is a fanatic....I guess its intuition....yeha nvm. Soon as I heard the Clan speaker go through the 2nd sentence, I realized....yeah he is a fanatic no point giving him a chance...pop pop..
