In the last large warehouse where you have to fight mercs 'n' mechs to get to Harkin at the other end, there is one platform roughly in the middle of the room that you have to vault onto in order to continue the mission. This platform has yellow edges, unlike all the other platforms, which have red edges. Once you and your squad are on this platform, you turn right and proceed a bit further. When you take a step down, Garrus says, "Aw, crap! Two heavy mechs!" This is when the two heavy mechs spawn. From there, you can retreat a couple of platforms, but you can't go all the way back to where you came from, because the platform with the yellow edges has been raised.

Last night, while I was playing this level, My team was still at the end of the warehouse where that fight begins, where Shepard opens the window, and he and Garrus eyeball Harkin and talk about Garrus's plans. Early in the fight - much earlier than usual - I heard Garrus say "Aw, crap! Two heavy mechs", even though we were a long way from the spot that makes the mechs spawn. I killed the two heavy mechs from long distance, and then walked forward. When I got to the platform with the yellow edges, it was already raised up. There was no way to vault up to it and continue with the mission. I had to reload. On my next playthrough, the level worked correctly.

Interestingly, at the time this happened, both of my squadmates were unconscious. Even though there were still enemies on the field and a fight was ongoing, Garrus revived himself so he could say "Aw, crap!"

I suspect that what fouled it up for that one playthrough is that one of my attack drones (I was playing as an engineer) wandered over to the "Aw, crap" spot that makes the heavy mechs spawn and raises the platform.
