Does anyone know how to find the last suit of juggernaut armour? I've found the fountain with the jug and altar but can not find the codex entry that gave me the specifics of the ritual.

From memory, I think this is the steps:
Take the jug from the fountain.
Fill it.
Put it on the altar.
Pray before the altar.
Take one sip from it.
Pour the rest back into the fountain.
Take the jug from the fountain.

I can't find a piece. I have 2 from the forest. One from the sarcophogus (once you pass the water/jug ritual).. That's 3.. I'm missing the 4th. I have feet, chest, hands.. and I'm missing the helm. The quest says I have recovered 3 items, but one still remains

The gravestone in the Western Forest was my last one. I guess it only works after you've done everything in the ruins (or maybe use another one first)

I put the entire set on Wynne since she's an arcane warrior. From what I gathered from the gem that unlocked this. The mage that hit the armor was the one who wore it

You want to start from the from the eastern gravestone first. The one where the 2 ogre's attack. The second one is close to the werewolve lair but not inside. Right about the entrance. 3rd one can be activated after these two which is on the western forest. The 4th item is inside the lair, which requires the ritual with the fountain. This will open the door that leads to the 4th and last piece.

Can anyone help me with this, I do these steps:

and then this happens
You kneel before the altar and prays. The water inside the jug suddenly begins boiling and bubbling until gass jets out of it into the air
... and then obviously the jar is empty and I can't sip from it!
Anyone have a clue how to do this?

Dunno if anyone is still stuck on this or not but I figured I'd post the fix for the evaporating water. You have to have the Carved Elven Tablet on you for the ritual to work. I hadn't grabbed it yet either. Here are the directions, as posted on another forum, to get to it.
Exit the altar room, take a right into a room with two rooms coming out of it from the north and south. I believe the southern room has a sarcophagus. There's a tablet in the sarcophagus, and once you pick that up, you can pray without having the water evaporate into gas.
