Fade Pedestal Bugged

I've been trapped in The Fade by the Sloth Demon. I collected all four forms and have defeated demons on 3 of the 5 islands, but now I cannot teleport. I can click on the Fade Pedestal and the "map" comes up, it won't respond when I try to click and teleport somewhere else. I'm effectively trapped in the first area where you get the mouse form. I've tried loading different saves and they do not work either. The only thing I did between my last play and now has been to open the DA Toolset.

I have the same problem, and i used DA Toolset too, but i didn't edit or save something.

Toolset installation fix
Sounds like toolset installations...
1) Load toolset, goto tool > export > Empty Export Directories
2)
FIND:
Windows XP: My Documents\\\\BioWare\\\\Dragon Age\\\\modules
Windows Vista: Documents\\\\Bioware\\\\Dragon Age\\\\modules
Delete anything called single player
3)
FIND
Windows XP: My Documents\\\\BioWare\\\\Dragon Age\\\\modules\\\\single player\\\\override
Windows Vista: Documents\\\\BioWare\\\\Dragon Age\\\\modules\\\\single player\\\\override
Delete anything in this folder
4)
FIND:
Windows XP: My Documents\\\\BioWare\\\\Dragon Age\\\\packages\\\\core\\\\override
Windows Vista: Documents\\\\BioWare\\\\Dragon Age\\\\packages\\\\core\\\\override
Delete anything in this folder

why are we doing that?

THe reason your doing that is to ensure nothing in any of the override folders is "Overriding" the core functionality of an object in game.
