I've been trying to start an Insane difficulty playthrough in ME 1, but I want to do that with an imported character that I used to beat Hardcore difficulty. Unfortunately, every time I try to change the diffculty on Eden Prime, it changes back to Hardcore, and of course there's no way to change the difficulty on an imported character before Eden Prime.

Am I doing something wrong? Do I need to use a fresh character to play Insane?

No, you should be fine with any character fresh or new as long as the achievement is unlocked.
