I don't understand, how has this happened? Her approval hadn't gone down any and it's at 100. What do I do?

Uhm... I tried to eliminate as much spoilers as I could, hopefully not. Er... why to be exact will she not "do the deed"?
The relationship is getting too intense for her, that's all I'll say.

Why wouldn't she share this with him? She just blew him off.

She explains it as best she can if you ask her about it afterwards.

I hate that part about Morrigan's romance too, but I wonder if a part of the reason she refuses to have sex with the Warden after she falls in love with him is because it reminds her of what is going to happen before the final battle when she will ask the Warden for the DR and then leave forever...
My interpretation is it becomes 'really real' to her. Up to that point she realized she had a duty and could have some indulgences, but at the end of the day she had something more important than even herself to do and worked towards that. Then she hits that point, the 90ish approval rating, where she realizes it's really, really going to suck and hurt to do what she has to do and start's trying to distance herself from it.
It's actually really amazingly done.

Its actually pretty interesting for a video game romance since the other Morrigan acts like an actual person with in accordance with her own motives and agendas and not simply someone for the Warden "to be with".

When your approval with Morrigan goes over 90 she won't do the deed with you anymore. Its part of her character arc and is intentional.
Not exactly. I've had her approval to 100 and sexed her for hours of game play.
Once you activate Flemeth, though, she comes to realize that she is bound to try to use you to do something unseemly. She feels guilty, so she won't sex you because she is trying to stave off growing any more intimate with you. It's the whole "If I stop having sex with him I'll stop loving him," routine. Which, of course, fails because she still loves you.
But in the end her "sense of duty" to do what she values most supercedes her love for you. She has a mission, and it must be completed no matter her feelings for you.
Side note: My canon character has a mission too - keeping Fereldan safe - and it supercedes my love for her. You all know where that goes.
