Okay, because I'm weird, and like doing things weird, there are a few missions that I'm purposely saving until after the SM. The Omega 4 thread has gotten much better since I last visited (kudos!) and I think I have a plan, let me know what you think -
Vents - Kasumi (L)
Biotic - Jack (L)
Fire Team (1&2) - Garrus (L)
Escort - Mordin (L)
Boss Fight - Kasumi (L) and Jack (L)
Hold the Line - Grunt (L), Zaeed (L), Garrus (L), Thane (L), Legion, Samara, Jacob, Tali.
If I'm doing it right, that puts me at a 2.0 HTL score, and everyone should live, yes?
You've missed Miranda from your list
That said, if she is Loyal then your plan will still work
I forgot to put her in, but if I'm doing the math correctly, even if she's not loyal I'll still have an HTL score of 2.0 = everyone lives.

I had no one escort the crew, they all died.
only tali (loyal) and mordin were holding the line, and tali survived.
Both crew memebers int he boss survived.
How come tali survived?

Unless the HtL score is zero (which is only possible when it consists of any combination of Jack, Kasumi, Mordin or Tali -- all unloyal), at least one person will survive holding the line. With only two squadmates holding the line and one of them being loyal, Tali survived.MrCry0 wrote...

Loyal: Grunt, Garrus, Miranda, Jacob, Samara, Tali, Jack
Unloyal: Zaeed, Kasumi, Thane, Mordin
Legion wasn't activated.
Tali went through the ducts, and then escorted the crew.
Miranda led the first fire team. Garrus led the second one.
Samara held the barrier.
Miranda and Jack went with me for the final fight.

I once went through it more as a thought exercise, so I don't know if this works in practice: Click Here. Seven for a crew of twelve. Three for a crew of eight.clennon8 wrote...
What's the max number of people you can have unloyal and still have everyone survive the SM, assuming you make all the right choices? I know it's at least 4. I recently went through it like this:

While all this is interesting I don't see how people just skip the loyalty missions. They are fun and action packed and basically just more of what makes the game so great, so why bail on them? I just finished my Hardcore run and had the ship ugraded, pretty much every weapon upgraded to 5/5 or 6/6 and everyone was loyal. Same result as my previous run, the entire team survived, I saved the entire crew and we just kicked ass through the entire endgame. It was actually easier this time, probably because I was more familiar with everything but still... It's pretty easy to keep everyone alive.
See, I'm not skipping the loyalty missions altogether, I just want to do it in a weird way. First off, I want to use Legion for parts of the game where I haven't seen him, and since I want to keep the crew alive, that means putting some loyalty missions off until AFTER the SM. Of course, since I do eventually want to do those loyalty missions, they have to stay alive for the quest to be active, so I use this handy dandy guide to keep everyone alive before the mission to keep max stuff available after the mission.

adding my 2 cents ;p
I went with the following set up
vents - Tali
fire team leader - Garrus
I took Thane and Jack as I opened the vents for Tali
escort - Jacob
2nd team leader - Garrus
Biotic - Samara
and again I took Thane and Jack
my final team - Garrus and Mordin
also I always start the suicide run straight after completing Legions loyalty quest, all my ppl were loyal etc

Btw, it strikes me that Jack (L) is pretty much a perfect choice to take along with you on your final team. She has Squad Warp Ammo (if you leveled her correctly) which is very useful against Collectors. Also, you're improving your Hold The Line score by removing one of the squishies from the equation.

Example. Right now with my latest Shepard I plan to make clan Urdnot as weak as possible before ME3. I'll probably kill Wrex, deny them Grunt, and leave clan Weyrloc and Maelon alone. Like Jack says... why the f- not? Sure, the less common ways to play the game might not even be recognized in ME3. But until we find out, I'll just make up the stories in my own head.

After that I tried to deliberately kill of the strongest squad members by sending in Garrus as the tech expert and Grunt as the second fire leader. At that point I only had Samara and Jacob as 2 point characters left so I let Samara get spirited away by the seeker swarms. Tali died during escort duty which left Kasumi, Mordin and Jacob to hold the line while I took Miranda and Zaeed to the end boss.
Result: Kasumi and Jacob survived and so did Shepard, despite a HTL score of 1,3

There is something that throws Ecael's calculations off when you get to 4 or less squadmates, but we don't know what it is.Hadeen wrote...

I've been trying to come up with a scenario when you have all 12 squadmates and achieve maximum carnage. This is what I came up with:
All 12 loyalty missions completed. Normal recruitment mission order.
-----------------
Final Squadmates: As few as possible.
-----------------
Loyals: Garrus, Grunt, Jack, Jacob, Kasumi, Legion, Mordin, Thane
Non-loyals: Miranda, Samara, Tali, Zaeed
Normandy Upgrades: None
Chakwas & Crew: Yeah, why not.
Before the Omega-4 Relay
- Side with Jack during the Miranda/Jack confrontation.
- Side with Legion during the Tali/Legion confrontation. Or ruin Tali's loyalty mission by handing in the evidence.
- Take the paragon path with Zaeed. Don't regain his loyalty.
- Ruin Samara's loyalty mission by scaring Morinth away. (Will result in a lost opportunity to get an Assault Rifle upgrade.)
Assignments
Oculus Fight - Take Kasumi
Vents - Jacob (dies)
Fire Team Leader - Any
Biotic - Bring Garrus with anyone (dies)
Second Fire Team Leader - Grunt (dies)
Escort - Zaeed (dies)
Final boss squadmates - Miranda and Samara (both die)
Distraction Team - Kasumi, Mordin, Tali
Deaths, before landing
Jack - No Armor Upgrade, laser'd
Legion - No Shield Upgrade, incinerated
Thane - No Weapon upgrade, impaled
Deaths, after landing
Jacob - Died by rocket to face
Garrus - Died by seeker swarm
Zaeed - Died escorting crew
Grunt - Died while closing second door
Samara - Died at final boss
Miranda - Died at final boss
-----------------
Final Squadmates: ???
-----------------
The HtL players are loyals Kasumi and Mordin with unloyal Tali. From previous testing, at least Kasumi is going to survive this scenario. However, with a HtL score of 0.66 (this is about as low as you could go), it might be enough to get Mordin and Tali to die. Which means Shepard might die in this scenario as well. I could not figure out any way to get a worse scenario than this with Samara being loyal.

Good way to redistribute those squad points so squad mates are optimal for those with the DLC

I haven't checked to see if doing the loyalty missions on the other hub worlds count against the crew death timer, but if they do, likely because they're main game missions rather than DLC.

The reason the LotSB doesn't activate the crew abduction or count against them is because the mission starts on a hub world, which you're free to visit after the crew's been abducted without it penalizing you. The whole mission starts without you ever leaving the planet, and then when you head to the Shadow Broker's domain, you still don't have a scene on the Normandy to worry about.

They count against other timers; for instance, I did both missions on Tuchanka, and when I got back to the Normandy that bastard TIM roped me into the Collector Ship for an hour of rape at Harbinger's hands. I know that I only had two missions left because I had loaded a prior save and done Zaeed's loyalty and the first Firewalker mission prior to doing Tuchanka instead.
Then I gamed the system

They really make it hard to fail the mission.
Makes me wonder if it is even possable for someone to play this game and NOT die on purpose.
