While from the story point of view it shouldn't be, Miranda and Jack are pretty much the greatest team you can have.
Doesn't the story suggest they worked as a team before already, even though not necessarily in that setup? Or were you thinking of the complicated situation because of their earlier... romance? relationship?
As far as the initial question is concerned, I personally find myself working with Miranda and Garrus a lot, really have to force myself to use others every now and then. I tend to leave both holding the line during the Suicide Mission though, which is where I learned to appreciate Jack, Samara and Kasumi (also great during her loyalty mission).

Who sucks donkey balls if you try to use them on the Derelict Reaper?
Garrus and Thane I'd guess, or Garrus and Morinth. Aliens are incapable of resisting Earth's donkey balls, it would seem.

It's all about what class your Shepard is.
Knowing Shepard's class would help in selecting squadmates that don't have complimentary powers.

I disagree because Flashbang Grenade is too powerful against husks. At the derelict reaper I would take Tali and Garrus.
For Tali recruitment take Samara and Jack maybe?
Armor Piercing Ammo + Shotgun + Concussive Blast nullifies this.
In Insanity:
For everything: Jack and Jacob/Morinth.
It will not fare too well against anything with protection on Insanity. Jack is a good character, but needs defenses stripped to do maximum damage. Jacob is well...Jacob, and Morinth has no defense stripping power unlike Samara. Vs. Barrier it is usable, but defininetly not optimal.

Grunt (limited powers), Jack (squishy, low dps weapon, no "stripping" powers), Tali (shotgun = meh, she dies too fast) Jacob (same as Jack just less squishy) are the worst imho. Note, this is comming from a vanguard on insanity, I imagine Jack and Jacob being very powerful on the easy settings because of the AoE CC when the enemies lack any protection. I also rarely take Kasumi as I find her dialogue very annoying in most situations, although she has great powers.
I always take Grunt for his squad incendiary ammo (on my vanguard because I don't like putting points in ammo powers when my squad can give me ammo powers) and for the fact that on Insanity he can do this.
