I've lost Miranda's loyalty in her fight with Jack. I didn't have enough paragon/renegade points to defuse the situation. Can't charm her either.
I did Miranda's mission first, then Jack's
But Miranda can still use Slam. I have the 1.02 patch installed.
Not really a problem but, anyone else with this issue ?
Miranda is unloyal and still has slam unlocked

I don't think they have to be loyal to unlock the power, just that their loyalty mission is complete.

If the character is unloyal, they won't have the alternate colors for their outfits. They'll still have their loyalty powers unlock.
