Never really liked Jack that much. If I have her in my party it is usually either with Garrus or Mordin.

I play vanguards and adepts... her squad warp ammo means I can use a non ammo bonus power (reav for vanguard and energy drain for adept)

Miranda (always) because of:
Example: a charge into 4-5 common enemies lined up. With mordin I can safely do it knowing of the panic effect bringing the number of enemies to maybe 2-3 next charge.
I might pick Zaeed for geth (Incineration bonus is no longer there) as concussive blast is very similar and with him I have inferno ammo (myself), disruptor ammo (Zaeed) and squad cryo (Miranda). I've started using Thane (AoE life/dual warp vs collectors) and Miranda (reave). I mostly ignore the rest.
- Squad Health: +7.50%
- Squad Weapon Damage: +15.00%

ME1: Garrus and Kaidan
Most of my playthroughs was as an Infiltrator and I was a Sabotaging, Overloading, Damping fool. Garrus and Kaidan added to my tech mayhem, plus Kaidan had my all-time favorite power Neural Shock. And the occasional biotic power.
ME1: Liara and Kaidan
When I played an Adept, my playthrough became all about the liberal use of Lift and Throw. With Liara and Kaidan on my team, Sovereign-Saren's ass never hit the ground once and it was the easiest end boss fight of my life. The only time they weren't used was during the Luna mission.
ME2: Legion and Tali
I am a Combat Drone loving fool this time. Set loose Legion's Combat Drone, and if that dies before the cooldown, then it's time for Tali's Combat Drone. And oftentimes I manage to get both out without them prematurely fizzing out. Plus, you know, Legion + Widow = teh awesome.
ME2: Samara and Thane
As an Adept, Samara and Thane became my go-to squad. Pull, Warp, Reave, Throw, Singularity. Everything a biotic team needs to cause mayhem. Give Thane his weapon bonus and a Viper sniper rifle, he's actually an extremely good sniper.
I think I do tend to vary my squadmates rather well in ME2, though. Legion and Tali tend to be the last squadmates I pick up, so really, I don't get a very good opportunity to use their Combat Drones aside from the suicide mission and whatever sidequests I've left for after the suicide mission. Jack probably gets used the least because her powers simply aren't good enough for me when playing on Insanity. Garrus, Miranda, and Kasumi probably get used the most. Garrus for sentimental reasons and Overload. Miranda because of Warp and Overload. Kasumi because she's awesome no matter what power I use.

Zaeed is a constant companion. Lately I've been pairing him up with Kasumi but also like the Zaeed/Grunt and Zaeed/Garrus combinations.

I like them all, for different reasons. Character/personality-wise, my favorites are:

ME2: Samara and Miranda
But these are role-playing preferences (i.e. friends to hang out with). For tactical favorites:
Against geth/mechs: Tali/Legion + Garrus/Miranda (Hack, Drone and Overload, Garrus preferred if Heavy Mechs)
Against mercs/vorcha/krogan: Samara + Zaeed (Incinerate and Reave)
Against collectors/harbinger/reaper: Samara + Miranda (Reave and Warp)

My go-to squad mates are Miranda, Samara, Garrus, Thane and Zaeed. I sometimes sprinkle in the others, except for Tali, who I only use on her loyalty mission (because I hate her and I don't care for her suite of powers).

In ME1 my squad is almost always Liara and Kaidan. Aside from the fact that I like their characters the best, I also like their powers the best.

ME1: Garrus and Wrex. Varied it at first, but in the final playthroughs they were just the ultimate duo to have with you.
ME2: Garrus and Grunt/Thane/Jack. I'd like to add Legion, but he's an option so late in the game, I don't really feel like I can count him.

I like to pick two characters that compliment my shep and use them in all situations. Personality is a big factor for me because I've found that you can basically beat almost any situation (yes, on insanity) just by being cautious (except characters that will close in to close range on you, like krogans and guys with flamethrowers). Beyond that it's just about efficiency (it's good to be able to 1) strip any kind of defense 2) have something to disable undefended enemies and instant kill husks)

Kasumi - amazingly deadly and versatile with flashbang, overload, and shadow strike. Takes a lot of managing because shadow strike removes any move command you had given her. I like her personality too.
Samara - basically a jedi, which put me off at first but when I saw that she's just following her code and not seeing things as good & evil I started to like her. Assault rifles, reave, and throw/pull make her good for pretty much everything.
Legion - good weapons, a shield, and overload essentially make him garrus 2.0 (new and improved! drone and hacking are just icing). Interesting character, too bad he shows up so late.
Miranda - I'll take her if i've got pull/slam or a squadmate with pull. She's useful and versitile but can't reach her full potential without another biotic.
Tali - Not very flexible but my first choice if I need someone to take down shields because I like her personality.
Thane - strong against everything except shields. Would probably be one of my favorite characters if he wasn't so amazingly boring that I can't take him anywhere.
Garrus - I like Garrus, but there's usually somebody else I'd rather take.
Mordin - sings Gilbert & Sullivan so he's alright in my book. I never really use him though because he's mostly good for incinerate (armor is the easiest defense to deal with).
Zaeed/Grunt/Jacob - strong fighters in general, but I find them kind of boring
Jack - her skills are useless against anybody with any kind of defenses and she doesn't have the skills to make good use of the shotgun (unlike all the other shotgun users she has no way to put up defenses and her crowd control doesn't work against enemies who haven't been stripped). Warp ammo is great though.

on my Soldier it was Liara and Kaiden, until Feros when Kaiden kept stepping in front of my sniper and absorbing the shots labeled for the Shiala clones, after Virmire it was a mix of garrus Wrex, and Tali/Liara and Ashley. for my Adept it was mostly Tali and Wrex, but I used Garrus and Tali for the final mission, zapping Geth left and right with Overload was fun.
